Documents requirements during application are the primary step for school admission. Therefore, you must meet all the eligibility criteria and application requirements for admission. Document requirements
1. Certified birth certificate
A student’s birth certificate is an essential document during admission to a Montessori school in Dubai. You must bring a copy with you. Moreover, students cannot proceed with admission without this document, according to the Illinois Missing Children Records Act, Section 325 ILCS 50. Parents must remember to submit it within 30 days of entrance. The Montessori school will accept a copy of the birth certificate and return the original to the parents.
According to the Missing Children Records Act, parents can submit an alternate document, such as Visa and Passport, if they have lost the child’s birth certificate. 2. Health Exam proof
From Kindergarten to 9th-grade admissions, students must get themselves tested and provide health exam proof. The document should suggest that the kid is immunized and fit for the school.
The health exam of a student should meet the below-mentioned requirements:
- A student must take the health exam within one year before being admitted to a Montessori school.
- State of Illinois Certificate of Child Health Examination should certify student’s health exam proof forms.
- Parents should sign the student’s Health History form.
Along with immunization, a student must undergo a complete physical exam routine. Also, schools may not approve IESA and IHSA forms for school sports. So, students can take a sports physical exam guided by the school.
The health exam should prove the following points:
- The students have TDAP doses required for 6th to 12th grade.
- The students have two Varicella doses, required for 6th to 9th grade.
- The students have two Mumps and Rubella dose, required for Kindergarten to 12th grade.
- Students must have a single dose of the Meningococcal vaccine needed for 6th and 7th grades.
Student caregivers can present health exam proof till 15th October for the current academic year if they cannot produce immunization proof. So, parents get 30 days to give health exam proof. All Dubai schools mandate that parents must provide physical and immunization evidence in the given timeframes.
Else, the school will exclude students who do not submit health exam documents.
- Eye exam proof
Following grade students must produce the eye exam document:
- K-8 grade students admitted at MSCU
- Kindergarten students
Eye exam must qualify under the State of Illinois Eye Examination. Additionally, an exam taken by the school is not enough, and students must get proof from a licensed ophthalmologist. Parents can take eye exams at the school only by 15th October if they cannot submit certified documents from an ophthalmologist. Remember to submit it within 30 days, or else the school will exclude your child.
- Dental exam proof
The following students must present dental exam proof:
- Kindergarten
- 2nd grade
- 6th grade
The State of Illinois Proof of School Dental Examination must qualify for the dental exam document. Parents must produce the dental exam proof by 15th May in school.
- School transfer documents
Students must get a copy of their previous school, allowing them admission to a new Montessori school. It is known as Authorization for the Release of Students Records.
- Emergency Medicine Authorizations
Kindergarten students with emergency prescriptions should provide the Emergency Medication Form to the school. The emergency conditions are Epi-Pen, Seizure, Inhaler, Asthma, etc.
